MAINTENANCE REQUEST FORM
FIRST - BEFORE YOU SUBMIT FORM, READ BELOW:
If you are reporting a WATER LEAK:
- Turn off the water at the supply source. If you cannot stop the flow or source of water, call our After Hours Emergency Hotline (808) 688-3300.
- Immediately call your assigned property manager OR THE AFTER HOURS emergency number.
- Use towels to soak up the water.
Before notifying Woodstock Properties, Inc. of your maintenance issue, 1st try the appropriate solution by clicking on: FAQ Maintenance. You just might resolve the issue, minimize any inconvenience or perhaps even prevent a tenant charge for the service call. If the maintenance issue is NOT resolved after trying our simple solutions, submit a Maintenance Request. We believe it's best to deal with small issues sooner than later since ignoring them can lead to the issue growing into a big problem. Please promptly report items in need of repair. The repair vendor will contact you to set up an appointment. Please arrange to meet the repair vendor at the scheduled time. Missed repair vendor appointments may be subject to a service-call charge.
By requesting this work to be completed, the tenant acknowledges that if the repair is found to be due to misuse or negligence, the tenant may be responsible for payment of the repair as per the Rental/Lease Agreement.

If the repair vendor, sent out by Woodstock Properties, Inc. fails to make contact with Tenant within 3 business days, Tenant agrees to submit a 2nd Maintenance Request Form.
